Engine fixes
- Game Over only if 0 lives remaining (allows 128-255 lives)
- Remove HUD flicker due to bad LCD IRQ timing
- Fast skipping challenge miss screen
Graphic tweaks
- Missing pixels in cloud outlines
- Missing pixel in large Mario's glove when jumping (NES)
- Missing pixel in fortune teller Mario card
- Wrong-colored pixels in Toad's House
